Your privacy is important to me.

A person’s sexuality is an extremely personal and sensitive topic that takes an extraordinary level of trust to share. Client information will be kept private and confidential, starting with our very first point of contact. The statement below is carefully worded to ensure trust in me as a professional and ease worry about sharing sensitive information with me. If you have any questions or concerns regarding confidentiality and non-disclosure, please do not hesitate to contact me for assurance and clarification.

I understand that I will be granted access to, or otherwise become acquainted with, some or all of the following information: personal thoughts/desires, visual observation of clients, clinical/medical information, billing information, scheduling information, and other information not listed. I understand and agree that except as required by law, I will use and hold all information in strict trust and confidence.

I acknowledge that it is my responsibility to respect the privacy and confidentiality of client information. I will not access, use, or disclose any confidential information, unless I am authorized or permitted to do so by law or as authorized by the client.

The only exceptions to this confidentiality statement are the following:

  1. If I am ordered by a judge during a legal proceeding.

  2. If I learn or believe you are doing or plan to do harm to yourself or others, I will take necessary steps to protect the safety of you and others.

  3. If I learn any information about current abuse or neglect of a juvenile person(s), person(s) with disabilities, or elderly person(s), I will take necessary steps to file a report with the appropriate officials or agency.

  4. There may be occurrences when I consult with peers and other outside sources about cases to ensure that I am receiving and providing the best assistance possible. In these cases, no personally identifiable information will be used.
